Declare that we have disclosed in a separate document accompanying this application, any medical condition or allergies that administrators and trainers should be aware of;
Are aware and in agreement with the rules and regulations of the academy as set out in the “Mellieha Athletic Club Academy Rules and Regulations” sheet;
